Next Level Basketball Vietnam — Liability Waiver, Consent & Release

I confirm I am the parent or legal guardian of the child named above. I understand that basketball and physical training carry inherent risks of injury, and I voluntarily accept those risks. I release Next Level Basketball Vietnam, its coaches and staff from liability for any injury, illness, or loss arising from participation, except in the case of gross negligence. I confirm my child is in good health and able to take part, and I authorize Next Level staff to arrange emergency medical care if needed. I give permission for photos and video taken during sessions to be used for Next Level Basketball Vietnam's promotional purposes. I have read and agree to these terms.
